Ethel Cain é o pseudônimo de um personagem fictício, criada pela mente de Hayden Silas Andhedönia. Em "Preacher's Daughter", encontramos uma vida inteira de traumas, experiências culturais e batalhas religiosas centradas na personagem de Ethel. É um álbum conceitual que explora temas como amor, sexualidade, violência, religião, família, abuso e como todos eles se relacionam. Ambientado em 1991, detalha a relação difícil e conturbada da protagonista com a religião, seu primeiro amor Willoughby Tucker, sua mãe (Preacher’s Wife), seu pai, e também consigo mesma. De forma linear, conta a história de sua família, seu primeiro amor, sua fuga de casa até o momento de sua morte, aos 21 anos.
